This post introduces a review of flap surgery for stage 4 ischial pressure ulcers.​

Ischial bedsore (stage 4)



A patient with a right ischial pressure ulcer visited Amun Plastic Surgery in Seoul, Korea.

A characteristic feature of bedsores in the ischial area is that the internal space is quite large.

The process of filling the large space inside even a small sciatic pressure ulcer with new flesh often requires a very painful period of several months or more.

Stage 4 ischial bedsore


For bedsores, the amount of space inside is more important than the size of the skin wound.

The internal space is continuously checked at the start of treatment and throughout the treatment process.​

Negative pressure wound therapy (NPWT) was performed before pressure sore flap surgery.


Negative pressure wound therapy can increase the success rate of pressure ulcer flap surgery.


This is because it takes time to fill the inside with healthy new flesh (granulation tissue) and check the results of the internal bacterial test.

Most pressure ulcer flap surgeries are performed under general anesthesia.

​At Amun Plastic Surgery, the procedure is performed under local anesthesia.

Most patients with bedsore have underlying diseases that make it difficult to administer general anesthesia.

In situations where it is difficult and burdensome for many patients with bedsores to undergo flap surgery due to the risk of general anesthesia,

Wouldn’t it be possible to perform it under local anesthesia? Starting with the thought

Bedsores are larger than they appear on the surface.

The deeper you go inside, the bigger it gets.

Below is a photo that allows you to estimate the actual size of the patient's bedsore.

In this way, the huge dead tissues in bedsore were completely removed.

This is the process I consider most important when performing flap surgery.

What is more important than actual suturing is 'How thoroughly do you remove the dead tissue of the bedsore?'

I am going into surgery with this thought in mind.

Taken immediately after flap surgery


The photo above was taken immediately after the flap surgery for stage 4 ischial pressure ulcers.

One blood bag (Hemovac) is placed inside the surgical site to prevent blood from pooling.

Blood bag lasts for more than a week.

Stitches begin to be removed from 2 weeks onwards.

1 week after flap surgery


The photo above was taken one week after the flap surgery.

The surgical site is healing well.​

The blood bag is still maintained.



The stitches will be removed starting 2 weeks after surgery.


Two weeks after the flap surgery, all stitches were removed.

The stage 4 ischial pressure ulcer healed well.

After getting all the stitches, I was able to use a wheelchair again.


Wheelchair boarding time starts at 30 minutes and gradually increases.
